This has been a progressing trend over several years, with data gathered from Clerkship OSCEs and Gateway, such as low physical exam scoring and evaluations from clerkship directors highlighting this issue. We recognize that COVID restrictions limited in-person clinical skills and practice, while variability among student learners and instructors also played a role. Nevertheless, we saw this as an opportunity to implement a standardized, condensed, head-to-toe screening physical exam, based on the work of Gowda and colleagues, which draws on insights from a national sample of clinical skills educators. Thus, in Fall 2024, the Art and Practice of Medicine course introduced the Core Physical Exam to the curriculum. This is a foundational physical exam that students can perform and replicate with every new patient. Using the core exam as a base, we additionally implemented Hypothesis Driven Physical Exam (HDPE) sessions in order to further advance clinical insight. These sessions encourage students to use clinical reasoning to determine which additional physical exam maneuvers are indicated for specific clinical presentations. Our overall objective in teaching the Core Physical Exam, with the HDPE methodology, is to ensure students have gained competence in the areas of physical exam and clinical reasoning.
