This has been a progressing trend over several years\, with data gathered from Clerkship O SCEs and Gateway\, such as low physical exam scoring and evaluations from clerkship directors highlighting this issue. We recognize that COVID restr ictions limited in-person clinical skills and practice\, while variability among student learners and instructors also played a role. Nevertheless\, we saw this as an opportunity to implement a standardized\, condensed\, h ead-to-toe screening physical exam\, based on the work of Gowda and collea gues\, which draws on insights from a national sample of clinical skills e ducators. Thus\, in Fall 2024\, the Art and Practice of Medicine course in troduced the Core Physical Exam to the curriculum. This is a foundational physical exam that students can perform and replicate with every new patie nt. Using the core exam as a base\, we additionally implemented Hypothesis Driven Physical Exam (HDPE) sessions in order to further advance clinical insight. These sessions encourage students to use clinical reasoning to d etermine which additional physical exam maneuvers are indicated for specif ic clinical presentations. Our overall objective in teaching the Core Phys ical Exam\, with the HDPE methodology\, is to ensure students have gained competence in the areas of physical exam and clinical reasoning. By practi cing these skills through repetition and consistency\, students will enhan ce their ability to provide comprehensive patient care. Objectives:At the c onclusion of this activity\, learners should be better able to:Identify ga ps in physical exam performance and clinical reasoning at a clerkship leve lDescribe the Core Physical Exam and its componentsDescribe Hypothesis Dri ven Physical Exam (HDPE) frameworkExplain the value of the Core Physical E xam and HDPE in medical educationCompare results of OSCEs from current M1 ’s and M2’s (exposed to Core Exam curriculum) to Clerkship OSCEs from M3’s (not exposed to Core Examcurriculum.)This activity is approved for 1.0* AMA PRA Category 1 Credit™ LOCATION:OUWB Lg Classroom A\, Administration Building\, Corewell Health W illiam Beaumont University Hospital END:VEVENT BEGIN:VEVENT SUMMARY:2025 Medical Education Month: The Real World - Standardized Patien ts Tell Real Histories DTSTART;VALUE=DATE-TIME:20250507T200000Z DTEND;VALUE=DATE-TIME:20250507T210000Z UID:140411768451 DESCRIPTION:Location: In person only\, OUWB Lg Classroom A\, Administrati on Building\, Corewell Health William Beaumont University HospitalPresente r:Leon Pedell\, MD Instructor\, Department of Foundational Medical Studie s\, Oakland University William Beaumont School of MedicineDescription:Firs t and second year students often complain of the lack of “real patient ” encounters prior to their clerkships.
